I just got in and it was pretty easy to fill out (I had the “service unavailable” message too the first time). Note the Medium is a 6 1/2 to 10 in Women’s.
This is the message I got when it finished processing:
Thank you, for your order was successfully received! Please allow 6-8 weeks for processing.
