1) You will need the proof-of-purchase tab from your Baby Einstein DVD Gift Set. Proof of purchase tab is located on back of DVD package just above the UPC code.

2) The rebate requires a cash register receipt from your Baby Einstein DVD Gift Set purchase imprinted with store name and dated between March 23, 2010 and May 31, 2010 with purchase price circled (photocopies are acceptable).

3) Cash register receipt from your Olan Mills photo sitting package purchase imprinted with store name and dated between March 1, 2010 and May 31, 2010 with purchase price circled(photocopies are acceptable).
